If you are a Grade 10 completer:

1. Fill out the application form and submit requirements (Photocopy of NCAE Result and 2 pcs Recent 2x2 Photo) at the Admissions Office

2. Pay Php 250 (for Career Assessment) at the Finance Office.

3. Admissions Office will give the schedule of Career Assessment and Counseling.

Note: Kindly secure your copy of the application form and receipt for the career assessment.

If you are a Grade 11 Transferee:

1. Fill out the Processing Form for SHS Transferee and submit a copy of your Grade 11 Report Card or list of subject taken from your previous school at the Admissions office.

2. On the provided schedule, go to the SHS Principal’s Office for your interview and assessment.

3. After your interview, visit the Admissions Office and submit your Processing Form for SHS Transferee and apply for your career assessment.

                Processing Form for SHS Transferee: Php 500

                Application Fee: Php 250

• Take the Career Assessment on your scheduled date
• You will be notified through a text message from BaliuagU on your scheduled Career Counseling (Release of Assessment Exam)
• You may also reserve your slot for Php 1,500*.
