For TRANSFEREE or SECOND DEGREE TAKER, scanned or soft copies of the following should be submitted.
- Transfer Credential/Honorable Dismissal
- Copy of Scholastic Record
- Certificate of Good Moral Character
- Photocopy of PSA Certified Birth Certificate
- 1 pc. 2x2 recent and decent colored picture with white background
B. Pay the Application Fee of Php 250.00 via GCash Biller.
