Kindergarten

• The child should be at least 5 years old on or before September 30, 2024 and in good health
• Certified copy of Birth Certificate
• 4 pcs. (1x1) recent photo with plain background

Grade 1 

• The child should be 6 years old and in good health 
• Certified copy of Birth Certificate
• 4 pcs. (1x1) recent photo with plain background

Grades 7-10

• Original Form 138 (Report Card)
• Original Certificate of Good Moral Character
• Photocopy of PSA Certified birth certificate
• 4 pcs. (1x1) recent photo with plain background
• Certificate of Ranking (for honor students)
• Recommendation Form from previous school (For Transferees)
