PROCEDURE
STEP 1: ADMISSIONS OFFICE
Fill out Admission Forms
Submit the following requirements:
- Ok for Admission Slip from Dean’s Office
- Transfer Credential / Honorable Dismissal (Original)
- Official Transcript of Records / Scholastic Records (Original)
- Certificate of Employment (Original)
- Marriage Certificate for Married Women (Photocopy)
- PSA Certified Birth Certificate (Photocopy)
- 1 pc Recent 1x1 ID Photo
STEP 2: GRADUATE SCHOOL (Room 105)
STEP 3: REGISTRAR’S OFFICE
- Claim the Assessment of Fees
STEP 4: FINANCE OFFICE
STEP 5: REGISTRAR’S OFFICE
- Claim the Printed Registration Form
Receive an email with Canvas credentials
Note: Only students who PAID the required fees and completed the above steps will be considered ENROLLED
