Enrollment Procedure for Graduate Studies New Student

ENROLLMENT PROCEDURE: GRADUATE STUDIES (NEW STUDENT)

PROCEDURE

STEP 1:  ADMISSIONS OFFICE      

Fill out Admission Forms

Submit the following requirements:

  • Ok for Admission Slip from Dean’s Office 
  • Transfer Credential / Honorable Dismissal (Original)
  • Official Transcript of Records / Scholastic Records (Original)
  • Certificate of Employment (Original)
  • Marriage Certificate for Married Women (Photocopy) 
  • PSA Certified Birth Certificate (Photocopy)
  • 1 pc Recent 1x1 ID Photo

STEP 2: GRADUATE SCHOOL (Room 105)

  • Subject Enlistment

STEP 3: REGISTRAR’S OFFICE

  • Claim the Assessment of Fees

STEP 4: FINANCE OFFICE

  • Payment of Tuition Fee

STEP 5: REGISTRAR’S OFFICE 

  • Claim the Printed Registration Form

 

Receive an email with Canvas credentials

Note: Only students who PAID the required fees and completed the above steps will be considered ENROLLED
